Please download the following PDF forms below IN ADVANCE of your arrival on the coast and have a set of forms filled out for EVERY member of your team. The Waiver and Volunteer Code of Conduct do not have to be signed in advance; we will have copies of these forms available at your group’s orientation. REQUIRED FORMS

Group Intake Form

One Group Intake Form should be filled out on behalf of the entire group by the group leader, whether your group is 50 people or only yourself.

All members of the group should fill this form out. Please be as honest as possible when assessing your skills

Parental Consent

To be filled out by the parent/guardian for all minors under the age of 18.
